RESUMO

OBJECTIVES: Ceftaroline, a broad-spectrum cephalosporin, has activity against Gram-positive and several Gram-negative bacteria (GNB). This study aimed to evaluate the antimicrobial activity of ceftaroline and comparators against isolates causing skin and soft tissue infections (SSTIs) and respiratory tract infections (RTIs) collected in Latin America (LATAM) in 2016-2020 as part of the Antimicrobial Testing Leadership and Surveillance program (ATLAS). METHODS: Minimum inhibitory concentrations were determined using both Clinical and Laboratory Standards Institute (CLSI) and European Committee on Antimicrobial Susceptibility Testing (EUCAST) criteria. RESULTS: Ceftaroline demonstrated potent activity against methicillin-susceptible Staphylococcus aureus (CLSI/EUCAST: MIC90 0.25 mg/L; susceptibility 100%), whereas activity against methicillin-resistant S. aureus varied for SSTIs (MIC90 1 mg/L; susceptibility 92.5%) and RTIs isolates (MIC90 2 mg/L; susceptibility 72.9%) isolates. For Streptococcus pneumoniae, particularly penicillin-resistant isolates commonly causing respiratory infections, high ceftaroline activity (MIC90 0.25 mg/L; susceptibility 100%/98.4%) was noted. All isolates of ß-hemolytic streptococci were susceptible to ceftaroline (S. agalactiae: MIC90 0.03 mg/L [SSTIs]; MIC90 0.015 mg/L (RTIs); susceptibility 100%; S. pyogenes: MIC90 0.008 mg/L; susceptibility 100%). Ceftaroline was highly active against Haemophilus influenzae, including ß-lactamase positive isolates (MIC90 0.06 mg/L; susceptibility 100%/85.7%). Ceftaroline demonstrated high activity against non-ESBL-producing GNB (E. coli: MIC90 0.5 mg/L, susceptibility 91.9%; K. pneumoniae: MIC90 0.25 mg/L, susceptibility 95.1%; K. oxytoca, MIC90 0.5 mg/L; susceptibility 95.7%). CONCLUSION: Ceftaroline was active against the recent collection of bacterial pathogens commonly causing SSTIs and RTIs in LATAM. Local and regional surveillance of antimicrobial resistance patterns are crucial to understand evolving resistance and guide treatment management.

OBJECTIVES: Ceftazidime-avibactam (CAZ-AVI) combines ceftazidime and a reversible ß-lactamase inhibitor that has shown activity against multidrug-resistant (MDR) Enterobacterales and P. aeruginosa. Using data from the Antimicrobial Testing Leadership and Surveillance program (ATLAS), this study examined the in vitro antimicrobial activity of CAZ-AVI and other antibiotics against Gram-negative bacteria collected from Chilean hospitals between 2015 and 2021. METHODS: Clinical isolates of Enterobacterales and P. aeruginosa were collected from three medical centres in Chile. Blood, abdominal fluid, urine, soft tissues, and respiratory tract samples were obtained from infected patients. Minimum inhibitory concentrations using the broth microdilution method were determined for susceptibility testing, and the Clinical and Laboratory Standards Institute (CLSI) breakpoints were used for interpreting the results. Extended-spectrum ß-lactamases (ESBL) and carbapenemase genes were also detected through polymerase chain reaction. RESULTS: A total of 2600 Enterobacterales and 836 P. aeruginosa were analysed. CAZ-AVI was the antibiotic with the highest in vitro activity against Enterobacterales (99.72%). The incidence of carbapenem-resistant Enterobacterales (CRE) was 1.5% (n = 39), and the antibiotics with the best in vitro activity were tigecycline (92.31%), CAZ-AVI (88.57%), and amikacin (79.49%). CAZ-AVI was the antibiotic with the best activity against ESBL-producing Enterobacterales (99.34%) and MDR Enterobacterales (99.31%). For KPC-producing Enterobacterales, susceptibility to amikacin was 100%, whereas susceptibility to CAZ-AVI was 91.67%. Regarding MDR and difficult-to-treat resistance P. aeruginosa, 44.83% and 38.99% were susceptible to CAZ-AVI, respectively. CONCLUSION: CAZ-AVI shows excellent in vitro activity against Enterobacterales in general, CRE, ESBL-producing Enterobacterales, and KPC-producing Enterobacterales. CAZ-AVI is also an option against MDR P. aeruginosa.

INTRODUCCIÓN: Ceftazidima-avibactam (C/A), ha demostrado reducir las tasas de mortalidad y el riesgo de nefrotoxicidad, comparado con colistin, la terapia convencional. OBJETIVO: Estimar la costo-efectividad de C/A versus colistin + meropenem en el tratamiento de infecciones por Enterobacteriaceae resistentes a carbapenémicos (ERC) en Chile. MATERIAL Y MÉTODOS: Se adaptó un modelo económico tipo árbol de decisión. Se utilizó la perspectiva del pagador público, un horizonte de tiempo de 30 días con extrapolación a la expectativa de vida. La información clínica se derivó de un estudio observacional. Los costos de los medicamentos y de atención corresponden a reportes locales. Los resultados se expresan como razón de costo-efectividad incremental (RCEI) por año de vida ganado (AVG) y por año de vida ajustado por calidad (AVAC) en pesos chilenos y en dólares estadounidenses (US$ 1,00 = $792,2218). RESULTADOS: Se obtuvieron 8,65 y 6,48 AVGs y 6,44 y 4,27 AVACs, para C/A y colistin + meropenem, respectivamente. La RCEI estimada de C/A fue $940.488 (US$1.187,2) por AVG y $938.715 (US$1.184,9) por AVAC. DISCUSIÓN: Dada la falta de publicaciones o evidencia, el modelo se basa en un estudio observacional. C/A reduciría la proporción de muertes e incrementaría los AVG y los AVAC, resultando en una alternativa costo-efectiva versus colistin + meropenem para ERC.

Resumen. En una población de niños Warao menores de 15 años con una tasa de prevalencia de tuberculosis (TBC) de 3190/100.000 se planteó como objetivo, estudiar ciertos parámetros de la respuesta inmunitaria en una población de 107 niños, de éstos, 32 presentaban TBC (27 PPD positivos y 5 PPD negativos) y 75 eran controles sin clínica de TBC (45 PPD positivos y 30 PPD negativos). Se evaluaron la inmunidad humoral, celular y algunos aspectos de la inmunidad innata. Entre los niños pacientes y controles, no se encontraron diferencias significativas en los componentes C3 y C4 del complemento. En cuanto a la inmunidad humoral, el grupo de pacientes presentó un alto porcentaje de niños con niveles totales de IgA e IgG elevados (13,8% y 79,3%, respectivamente) en comparación con los niños controles (0% y 69,2%). En el grupo control, un alto porcentaje de niños presentaron niveles totales de IgM e IgAs elevados (69,2% y 56,16%, respectivamente) en comparación con los pacientes (48,3% y 31,25%). El 100% de los niños presentó altos niveles séricos de IgE. Los estudios de la respuesta celular retardada mostraron que los niños de mayor edad presentaron las mayores medidas de induraciones. Los niños menores de 7 años presentaron similar reactividad positiva al PPD independientemente de la presencia o no de cicatriz de BCG. Un alto porcentaje de niños sin y con cicatriz (46,8% y 27,6% respectivamente) presentaron medidas de induraciones a la tuberculina de 0 mm. La reactividad a Candida mostró un 80% de niños con estados de anergia no específica. Sin embargo, el mayor porcentaje de niños con TBC estaba vacunado y presentaba positividad a la prueba de tuberculina. En conclusión, en los niños Warao se observó un aumento en los niveles de los componentes C3 y C4 del complemento y una hipergammaglobulinemia. Los resultados de los isotipos mostraron que, la disminución de IgAs podría ser un marcador de enfermedad activa, mientras que el aumento de IgM un marcador de infección reciente. La policlonalidad fue independiente de la reactividad al PPD.

A significant number of children without or with scars (46.8% and 27.6%, respectively) showed induration values of 0 mm to tuberculin skin test. The Candida reactivity showed a high percentage of children (80%) with anergy status. In conclusion, an increase in the levels of complement components C3 and C4 and hypergammaglobulinemia was observed in Warao children, and these results were independent from PPD reactivity and BCG vaccination. The isotype results showed that the decrease in sIgA could be and active disease marker, while the increase in IgM levels could represent a marker of recent disease.

Tuberculosis (TB) is a public health problem in Venezuelan indian communities. This study describes differences in hematological parameters in TB patients from two populations, Warao indians (WP) and creoles (CP). Control groups included Warao adults (WC) and children (WCh) as well as creole adults (CC). The results showed that the WP and WCh had significantly lower hemoglobin (Hb) levels than the WC and CC groups. Warao patients and two Warao control groups had significantly lower concentrations of median corpuscular Hb (MCHC) than the CP and CC groups. Regarding leucocyte indices, we observed a significant increament in the monocytes percentage in WP, WC and WCh, compared to CP and CC. In contrast, the neutrophils percentage was significantly reduced in all the Warao groups compared with the creole groups. Both WP and CP patient groups had higher platelet counts than those of the WC and CC groups. In relation to the body mass index (BMI), used as nutritional status indicator in Warao indians, we observed that the highest BMI was obtained in the adolescent group; with a continuous decreased with age. In conclusion, both WP and WCh presented decreased levels of Hb with erythrocytic hypochromia. Similar to the WCh, indian adults, even in the absence of the disease, presented a formula of white blood cells characterized by monocytosis, high percentage of lymphocytes and low percentage of neutrophils.

La tuberculosis (TBC) es un problema de salud pública entre las comunidades indígenas de Venezuela. Este estudio muestra los valores de los parámetros hematológicos de dos poblaciones diferentes con TBC, los Warao y los criollos. Se estudiaron pacientes Warao (PW) y criollos (PC), controles adultos tanto Warao (CW) como criollos (CC) y niños Warao (NW). Los PW y los NW presentaron valores significativamente bajos de la hemoglobina (Hb) en comparación con los CC y CW. Todos los grupos Warao presentaron concentraciones significativamente disminuidas de los valores de la concentración de hemoglobina corpuscular media (CHCM) en comparación con los PC y CC. En cuanto a los glóbulos blancos, los grupos Warao, incluyendo los niños, presentaron un incremento significativo del porcentaje de monocitos y linfocitos en comparación con los PC y CC. Sin embargo, el porcentaje de los neutrófilos se encontró significativamente disminuido en comparación con ambos grupos criollos. Ambos grupos de pacientes así como los niños Warao presentaron incremento significativo en el número de plaquetas en comparación con los CW y CC. Los resultados del índice de masa corporal (IMC) en la población Warao, el cual fue utilizado como indicador del estado nutricional de la población mostraron que, éste alcanzó el máximo en el grupo correspondiente a la edad de los adolescentes y luego disminuyó a medida que aumentaba la edad de los individuos. En conclusión, tanto los PW como los NW presentan disminución de los valores de la hemoglobina, la cual se correlacionó con una hipocromía eritrocitaria. Al igual que los NW, los adultos Warao, aún en ausencia de enfermedad presentaron una fórmula leucocitaria caracterizada por monocitosis, aumento del porcentaje de linfocitos y disminución del porcentaje de neutrófilos

It is difficult to establish a definitive diagnosis of tuberculosis in rural areas where there is no access to a large hospital. The Warao people of the Delta Amacuro State in Venezuela, have a very high prevalence of adult TB, and we suspected that the Warao children would also have a high prevalence of the disease, almost entirely undiagnosed. We applied a simple methodology to select children suspicious for tuberculosis that is based on a rating system using clinical criteria, reactivity to tuberculin and intradomicilliary contacts. Of the 502 children under the age of 15 that were evaluated with this rating system, 27 were determined to be suspicious of TB and were further evaluated by a chest X-ray. Radiologic confirmation of TB was found in 16 (60%) of the 27 suspicious children. Of these 16 patients, 13 (81%) were PPD positive and 3 were PPD negative. Additionally, 7 of the 16 children with pathologic x-ray changes had one or more confirmatory findings: 3 were positive by culture or smear examination and 5 had a positive serologic B diagnostic test. In conclusion this methodology proved to be highly efficient in diagnosing childhood tuberculosis in this population, and should also be useful in other rural populations with a high prevalence of adult TB.
